Please get admission advice along with desired category of room opted, recorded in the OPD book from your doctor.
Please request your doctor to provide Estimate cum Undertaking Form in duplicate. Retain one copy with you. Other copy duly signed by the patient / attendant shall be attached in the OPD Medical Record File.
To avail pharmacy credit, deposit 100% of the estimated pharmacy cost initially. After 90% of the initial deposit is utilized, get revised estimate form from your doctor and again deposit 100% of the estimated pharmacy cost.
Please deposit advance for any investigations / procedures / treatment as per hospital policy, at the time of admission.
Passes issued to the attendants are on returnable basis, at the time of discharge at the billing counter. In case the pass is lost and a duplicate pass is to be issued an amount of Rs. 100 will be charged for the same. (IPD Admission only)
